We have AlwaysOnTop for the Browser set to True. Should this prevent a user from ALT tabbing to the desktop on Windows 7?

No, it sets PWB to be the top most window. This does not prevent Alt-Tabbing.
To prevent access to the Windows Desktop, enable PWB Secure Mode, or use HideGUI (available on the downloads page) to disable aspects of the Windows Graphical User Interface.
